SERVICE AGREEMENT

This represents an agreement between you, the “Client” (or stated as “you”), and Carole Walker (doing business as Simple Transformations – www.SimpleTransformations.ca).

Appointments

Please contact Simple Transformations (either online, email, or by phone) to set up an appointment for a counselling session with Carole Walker. If you do not attend the scheduled appointment and failed to provide 24 hrs notice then a cancellation fee (the cost of that session) will apply. If you’re unable to make your regular appointment or booked session, please contact Carole Walker as soon as possible, as she may be able to offer you an alternative session that week, subject to availability.

Carole Walker commits to offering you regular appointments, usually on a weekly basis, because of the importance of maintaining the therapy conversation in ongoing and continuous sessions. Many people ask how many sessions they will need.  There is no minimum and there is no maximum number of sessions with Carole Walker. However, research has shown that couple therapy takes from 8-20 sessions.Carole Walker tries to accelerate the process by assigning homework to practice outside the session. The timelines is always dependent on the level of commitment of each partner.

Payment Methods

Clients can pay for their sessions by cash, electronic money transfer or cheque payable to Simple Transformations prior to each session. Credit card payments will soon be available.

A session fee is paid by the Client at time of service. A receipt is provided for Client submission to employee assistance program or insurance provider of benefit plan. (Please verify that your insurance company will cover counselling services provided by a Registered Social Worker).

Exceptions for Clients referred by the following Employee Assistance Programs and Health Benefit Insurance Companies

Confidentiality

All sessions are confidential. However there are two exceptions which all mental health providers have a duty to report: If you fall in the two high risk categories of hurting yourself or hurting another. Also, in couple therapy, what is discussed with one partner individually is kept confidential and not shared with the other partner by the therapist. The sharing of information is encouraged between the couple.

All sessions are spaced to allow for confidentiality between clients.

You can be assured that we will respect and never share your personal information. According to the ethical codes of our professional bodies, we will keep your personal and/or financial details safe and will not divulge any of your information.
